By entering the information requested in the fields below, I ("Signer") indicate my understanding and acknowledgement of the guidelines, and acknowledge that the organization ("User") named below irrevocably and unconditionally agrees to defend, indemnify, and hold harmless the Daniels Fund and each of its officers, directors, associates, and agent, from and against any and all claims, liabilities, losses, and expenses arising from, or in connection with, any act or omission associated with use of the Daniels Fund Meeting Space and meeting rooms. I understand and acknowledge that I may be liable for cleaning and/or damage fee(s).

By entering the information requested in the fields below, I (“Signer”) indicate my understanding and acknowledging of this document, and acknowledge that neither the Daniels Fund nor any of its and each of its officers, directors, employees, agents, and funding partners, warrants or represents that the Daniels Fund facilities are suitable for use by the Organization (“User”) for its intended purpose or are free from defect or danger. The Organization uses Daniels Fund facilities at its own risk. The Organization is solely responsible for the acts, conduct, behavior and actions of its employees, agents and invitees. To the fullest extent permitted by applicable law, the Organization is responsible for any loss or damage it or any of its officers, directors, employees, agents or invitees suffer while traveling to or using the Daniels Fund facilities. I understand and acknowledge the Organization’s contractual responsibility to use the facilities in a responsible manner, to leave the facility reasonably clean and free of damage and to reimburse the Daniels Fund for costs associated with cleaning and/or damage.
